Yes, a valid prescription from your physician is required.  The prescription can be faxed to 212.202.6379, or a scanned version can be emailed to customercare@makemefertile.com.

    * It is for your personal use and not for commercial resale
    * The supply is for up to 3 months
    * You have a valid prescription from a registered physician
